A Review of Lost Fishing Vessels & Crew Fatalities, 1992 - 2010
Shortly after the 1999 Fishing Vessel Casualty Task Force report was released, industry and senior Coast Guard managers requested more details about fishing vessel casualties. The Compliance Analysis Division (now CG-5452) collaborated with the Fishing Vessel Safety program manager and prepared a follow-on review to provide information about why and how such incidents occurred. That report was distributed in October of 1999. This document is the fifth edition of the casualty study with newly added data for calendar years 1992 through through 2010.

Parasailing Casualties
Two people died in a parasailing accident off the coast of Ocean Isle Beach, North Carolina on August 28, 2009. A one-man formal investigation was convened by Coast Guard District Five to determine the causes of this incident and determine what actions can be taken to prevent similar tragedies from occurring in the future.

Report of the Coast Guard-AWO Bridge Allision Work Group,
On May 26, 2002, a tow struck the I-40 highway bridge over the Arkansas River. The bridge collapsed, resulting in the tragic loss of the lives of 14 motorists. Under the auspices of the U.S. Coast Guard-American Waterways Operators (AWO) Safety Partnership, the Coast Guard and AWO convened a work group to investigate the prevalence and causes of bridge allisions involving barges and towing vessels and develop recommendations to prevent allisions and mitigate their consequences.

USCG / AWO Review of Crew Fatalities 2000-2009
Coast Guard casualty data shows that crew fatalities in the towing industry have occurred at a rate of approximately 11 per year, over a recent 10 year period. Although there has been some year-to-year variation, the data suggests that the fatality rate will remain constant unless additional preventive measures are implemented. Consequently, the USCG/AWO National QSC requested a review of those fatalities, in order to identify the most significant factors. This report describes the results of that review.
